Have a great week!

Kudos

Kudos this week go to Dan Turner from Corey Bradley for covering her patients when she wasn't feeling well and for an all around fun two weeks on the teach service together! Leah Machen sends kudos to the Duke Family who took care of her when she was very ill last week: Elieth Martinez, Ashley Spann, Juan Magana, Kevin Smith, Sipa Yankey, Stephen and Raha Kimani, Lauren Sharan, Sky Vanderburg, Adam Banks, Alyson McGhan, and Lucy Cauthen at the VA. Also to Linda Koshy and Phil Lui who covered Leah's Duke night float shifts and Richard Lee, who completed discharge summaries on Leah's behalf. Kudos to Lauren Collins and Josh Briscoe for excellent SAR Lectures, and to Philip Liu for a great chair's conference.

PubMed from the Program

This week's pubmed from the program goes to Robert Diep for the acceptance of his manuscript entitled "Pregnancy in patients with thrombocytopenia due to thrombocytopenia absent radii (TAR) syndrome" for publication in Annals of Hematology.

Clinic Corner: PRIME

Contributed by Leigh Wynkoop

If you were in clinic last week, you participated in our latest clinic flow study, designed to see how long every step of a visit lasts, from:

Lobby checkin --> nursing check in --> your initial time in the room --> time with preceptor --> closing time in room --> nursing/lobby checkout.  We've been aware that there can be hangups in the process but, spurred by your survey input, we proceeded to implement a flow study to get some data to illuminate exactly where the problems lie.

Speaking of which, those of you who replied to the survey, thanks for your input.  We have discussed them with the attendings and staff and are grateful for constructive feedback which can improve your experience.

REMINDER: At every visit, please renew all medications to get the patient easily to their next appointment.

Lync has been replaced by "Skype for Business" which might not automatically open when you log-in the first time, so be sure to get that up and running. Vetlink is still unreliable and we are awaiting a patch from national, so you'll need Skype to be informed patients are here if Vetlink is not working.

Hard to believe we are saying good-bye to our crop of PRIME folks moving out into the world!  Thank you so much for your service to our clinic and especially to our Veterans.  We happily accept any news updates from you either in person or by email if you want to tell us how things are going in your next phase of career and life!

Clearing the VA - Some reminders for outgoing house staff

As you move on to accomplish other great things, there are a few items we would like you to complete on or before your last day of your last rotation at the Durham VAMC.

  1. Progress Notes/Discharge Summaries - Please make sure to sign (co-sign) or delete (if not needed) any progress notes and/or discharge summaries.  Any left unsigned will have to be reviewed by the Section Chief or Chief Resident for disposition.
  2. Alerts - Please clear all active alerts prior to leaving.  Also assign a surrogate in CPRS to receive any future alerts so that we can ensure any urgent clinical issues are addressed.  If you are unsure of who to assign as a surrogate, please talk to your section chief or Chief Resident (PRIME residents should speak with Dr. Patel or Leigh Wynkoop).
  3. Personal Lists - Please delete all personal lists created in Vista/CPRS (and remove yourself from any team lists).  If these lists are left, they will continue to generate unnecessary alerts that will go to your surrogate and either the surrogate, Section Chief, or Chief Resident will have to review before they could be deleted.
  4. Sticky Notes - Please review, resolve, and delete any "sticky notes".  These notes can only be seen by you so if there are actions that need to be taken after you're gone please document in the record or hand-off to a continuing clinical staff such as a team resident/fellow, mid-level, or other attending.
  5. PIV Card Reader - If you were issued a PIV Card Reader, please return it to me.  If I am not available, you can leave it in the Medical Service Office (C8017).

Thank you for your service to the Medical Service and Durham VAHCS.  We wish you only the best in your future endeavors!
