The viewpoint provides an overview to date of CMS’s efforts to transition fee-for-service Medicare into an alternative payment system that rewards high quality and efficient use of resources.  The authors describe early results of important tests, key issues to observe over the coming years, and conclude that the success of efforts to transition Medicare fee-for-service will largely be dependent on external stakeholders, particularly physicians, to ensure the program remains fiscally sound so that it will continue to pay for high quality care for tens of millions of beneficiaries.
