From the Director

Thank you to Dr. Cooney (as well as Elizabeth and Brittany) for a fantastic residency party on Friday night! Great turnout, great call to move inside (monsoon, anyone?) and overall a really great time! 

This week, we are looking forward to the PWIM welcome event for residents and fellows to close out the summer party season. Don’t worry, we will have continued events, including the first division sponsored Thursday get-together (thanks Hospital Medicine!) later in September, an upcoming trivia night, Sundays in the park with Dr. G, and more.

Congratulations to all of our graduated SARs who have taken the ABIM exam in the past few weeks! We also kicked off our 2019-20 Learning Health Systems Training Program this week, with new residency members Jess Regan, Austin Wesevich, Claudia Leung, Callie Berkowitz, Sean Doerfler, Qasim Hussaini, Vinay Choksi and Bhavik Patel as well as fellows Hope Weissler, Anthony Carnicelli, Adam Nelson, Andrew Johanemann, David Schleintz, Jessica Ma and Philip Chu! Great group, should be a great year!

Rising SARs (that’s you current JARs!), the applications for short term global health rotation will be out soon and due Sept. 30! Be on the lookout for a description of all elective opportunities for SAR year this week!

Many congrats and kudos! Congratulations to Jordan and Audrey Gales on their wedding, to Josh and Alyssa Lee on their wedding and to Payton Kendsersky and Jon Reichstein on their engagement!! Congrats also to Duke IM grad Jessica Morris on her wedding as well! Also kudos this week to Trent Wei and Tsion Aberra for volunteering as “patients” in our 9th annual Physical Exam Week, to Sonali Bracken from John Davy for excellent teamwork and responsiveness in the VA MICU, to psychiatry intern Ray Haider from Mike Noujaim for outstanding work at DRH, to Kyle Geurink for being a very supportive and helpful call buddy on a busy night and to Madhu Eluri and Elizabeth Ockerman for being amazing MICU colleagues this month from Sonali Bracken, to Tsion Aberra for her extremely hard work on a busy GM service from Dana Clifton, to Yoli Guzman from Liz Hankollari and to Kim Darlington from Aubrey Jolly-Graham for outstanding NF patient care, and to Becca Lumsden being a huge team player doing multiple jobs in the CCU from Matt Labriola!

We are excited to hear about all the interviews for our fellowship applicants.  Please keep updating the interview spreadsheet so we can keep track of everything – much appreciated.

Clinic Corner: Ambulatory

Ambulatory Threads
We hope the threads are going well, and we appreciate everyone’s patience as the new year brings occasional changes and surprises to schedules. Thread schedules are very complex. Caroline has been teaching everyone how to look up the clinic of the attending you’ll be working with in advance – please check with Caroline if you are not yet doing this!

Ambulatory Lab Routing Changes
Residents at the DOC and Pickett have been doing a great job adjusting workflow as a result of the new DUHS system wide changes which are aimed at addressing patient safety issues. As a reminder (particularly at the DOC):

  1. Upon signing in to your workstation in the beginning, DO NOT select an attending, leave it blank
  2. Within each encounter, PEND all orders until you know who you are signing out with. This is critical. In the past, the name you put in that field didn’t matter, since results routed to a default attending designated at the front desk on check in. No longer!! The attending you put in AT THE TIME OF SIGNING ORDERS will be the attending copied on the results.
  3. This is true for POC orders, and all orders (because the attending designation prompt only appears ONCE per encounter) . Luckily, at the DOC, nurses are now doing POC glucose and POC A1c without our order, per protocol. But other POC orders will need to wait until you sign out. You can sign these selectively, first, as soon as you start signing out, to get things moving! (Selective sign is tiny, locate in the lowest right hand arrow, to the right of signing orders… try it!!)

Observed Encounters
Reminder to check in with your attendings every time you are in clinic to see if it’s a good opportunity for observed encounters! The goal is TWO for every Intern, ONE for every JAR and SAR, during the year. Please help us get these done by being proactive during clinic – asking us for observation before you see a patient. Thanks in advance for helping with this!!

Review clinical pearls from resident report on the ACR Teach Blog!
On many rotations residents generate excellent clinical teaching points that the ACRs send out for residents on that rotation. Now, this high-yield information will be available to everyone via the ACR Teach Blog! As part of the new Duke General Medicine Teaching Service website, the ACRs will regularly update the ACR Teach Blog with valuable information in case you’re on a different rotation. Feeling down? Need to talk to someone? Opportunities for Wellness
All trainees at Duke have FREE access to Personal Assistance Services (PAS), which is the faculty/employee assistance program of Duke University. The staff of licensed professionals offer confidential assessment, short-term counseling, and referrals to help resolve a range of personal, work, and family problems. PAS services are available free of charge to Duke faculty and staff, and their immediate family members. An appointment to meet with a PAS counselor may be arranged by calling the PAS office at 919-416-1PAS (919-416-1727), Monday through Friday between 8:00 A.M. and 5:00 P.M. For assistance after hours, residents and fellows can call the Blood and Body Fluid Hotline (115 inside DUH, 919-684-1115 outside) for referral to behavioral health resources. Another resource is Duke Outpatient Psychiatry Referrals at (919) 684-0100 or 1-888-ASK-DUKE. https://www.hr.duke.edu/pas/
