From the Director

First, sorry Jason, but GO RAVENS! A huge thank you to Erin Payne and Dave Butterly for a fantastic recruiting season. It's a long haul and you were terrific. Also thanks to Randy, Jen, Laura, Lauren, Sheila and Shawna for early mornings, late evenings, set up, clean up, snacks, moral support and keeping the rest of the program running from November to February.  And, thanks to all the residents for helping the applicants understand how amazing you are! Glad many of you could join us at the end of recruitment celebration on Friday. Kudos this week go to Marcus Ruopp for outstanding work on ID consults (from attending David Holland), to Scott Westphal for his gen med interns for great teaching and team leadership, to Newton Wiggins for coming in early to help Erin with some second look applicants, to Nilesh Patel for making some very constructive suggestions at the DOC (from Lynn Bowlby) and to Blake Cameron and Natasha Cunningham for spectacular SAR talks.   Also big kudos to John Stanifer for his international Society of Nephrology Global Outreach and Prevention grant to study "Chronic Kidney Disease Awareness, Screening and Prevention Among the General Population of the Kilimanjaro Region, Tanzania". Way to go, John! This week is the NC ACP meeting, and we have a lot of posters! Our medical jeopardy team should do quite well, and Jason Webb will be presenting in the chief resident competition.  We look forward to great Duke representation. During GME week this week, Bill Iobst from the ABIM is here to teach about milestones and assessment. Want a sneak peek at how we will be assessing your progress as a physician next year? What Did I Read This Week (by Murat Arcasoy, MD) 

 Recommended Adult Immunization Schedule: United States 2013 Annals of Internal Medicine 158:191-199, January 29, 2013    [/box]   

Why I read this article:  In my practice, I vaccinate many of my patients with chronic hematologic disorders or I am frequently involved in the decision when they are vaccinated by their primary care physicians. The recent pertussis outbreaks across the country have raised concern in the public and health care providers, and I would like to adhere to the latest guidelines. What did the authors do:  The Advisory Committee on Immunization Practices reviewed and updated the adult vaccination schedule. It was noted that current levels of vaccine uptake for adults are low and that a strong recommendation from a vaccine provider is associated with increased uptake. Results and conclusions:  The article has two figures of the recommended adult immunization schedule by vaccine, age group, and medical or other indications. Live vaccines (such as the attenuated influenza, zoster…) are avoided in persons with immune deficiencies or immunocompromising conditions.  This situation applies to many of my patients with hematologic malignancies and those on immunosuppressive treatments. Some of the highlights of the updates are: 1-The 13-valent pneumococcal vaccine (PCV13) is recommended for adults aged 19 or older with immunocompromising conditions (chronic renal failure, nephrotic syndrome, functional or anatomic asplenia, CSF leaks, or cochlear implants) who have not previously received PCV13 or PPSV23. This first dose of PCV13 should be followed by PPSV23 at least 8 weeks later. Adults age 19 or older with the conditions mentioned above who have received 1 or more PPSV23 doses previously, should receive PCV13 one or more years after the last dose of PPSV23. 2-Tetanus, diphtheria, acellular pertussis (Tdap) is routinely recommended in adults aged 65 or older and for pregnant women (during 27-26 weeks’ gestation) with each pregnancy (based on optimal protection for the woman and infant in the first few months of life). Trivia Night (Submitted by the Rankin Stead Society)

The Rankin Stead Society invites all Medicine residents, regardless of Stead group, to a special Trivia Night to be held on Wed, Feb 20, at the Carolina Ale House (3911 Durham-Chapel Hill Blvd).  The event will benefit a charity chosen by the Stead society that musters the highest trivia score.  The Stead groups will begin forming tables at 7pm and will cover your food and drink for the evening.  Trivia starts at 8pm.  To the highest scoring Stead team, we will provide $200 to donate to their local charity of choice.  Please come out and have fun for a good cause! In his role with the ABIM, Dr. Iobst has developed outreach activities for residency and fellowship training programs, established faculty development opportunities, worked to enhance assessment methods for competency evaluation, and has assisted in the development of the Internal Medicine Milestones.  Below are the workshops and opportunities for hear his presentations:
