NEJM-FSGSReview
This review of FSGS appeared in the December 22nd issue of
NEJM. This week we've had 2 patients on the consult service with Nephrotic syndrome so I pulled it for my team. The authors are prominent researchers in the area of glomerular disease and have published extensively in this area. Like all of the reviews published in the
NEJM it is well written and gives the reader a broad overview of the pathophysiology, clinical presentation, and the treatment options available for FSGS.
FSGS is a leading cause of nephrotic syndrome in our patient population and is the most common primary glomerlar disease leading to ESRD. The incidence of FSGS is higher and the rate of renal survival is worse in our African American patients. Most cases are primary however FSGS is seen in viral infection (HIV, EBV, Parvo) or related to drugs (heroin, interferon, pamidronate). There are also specific podocyte gene mutations which have been associated with familial FSGS.
Dr Michelle Winn and her group here at Duke have described a mutation in the TRPC6 channel associated with familial FSGS and her work is referenced in # 25 and 5 in this article.
The genetics of the disease along with an overview of podocyte biology is covered. The disease also poses a problem for the transplant community with a recurrence rate of 30-40% after transplantation. Therapeautic options are summarized and a treatment algorithm is included on page 2408.

Although we did NOT reach our goal of 100%, we achieved quite an admirable performance. Neuro and Derm narrowly edged us out of 1st place (who have much smaller programs), but we clearly had the best response rate for any program >20 residents. Excellent work to all (except the 26 of you who did not respond)

Other Important Reminders
- Schedules - A reminder that schedules are prominently posted in MedHub, including detail schedules for CAD, CCU (Duke and VA), CCU Intern, MICU (Duke and VA), Duke Night Float-SS, Gen Med (DRH, VA, and Duke), Neuro, Pulm, Hem Onc. We would recommend reviewing these documents prior to starting rotations on these services so that you are fully aware of your assignments. The DOC Call Schedule has also been recently added to the list.
- ACR Applications are to be submitted to the Chief Residents no later than Tuesday, January 3rd
- Clinical Research Course Applications are to be submitted by Tuesday, January 3rd
- Ambulatory Care Leadership Track Applications are due by January 8th
- SAR's - End of Year: If you will be leaving Duke at the end of the academic year it is not too early to think about/discuss travel plans with the Chiefs. We will consider your requests, but in doing so, please be aware that we must meet certain training requirements, and that last minute changes/requests are the most challenging or in fact may not be possible to accommodate.
- IM Boards - February 1, 2012 is the deadline to register to take the boards
Hospital-onset Clostridium Difficile
