From the Director

Hey team! Three interview days down - thanks again to everyone who has helped to show our future residents how amazing our program is. They have repeatedly commented on your camaraderie as well as how impressed they are with report and chair's conference - way to go. Of course, everything else continues as we add in recruiting. Thanks to Ed Coverstone, Jason Watts and Lauren Gratian for some great report cases (constrictive pericarditis, awesome!). And to our VA and Duke gen med teams for working through some very busy times.  And of course Talal Dahhan with guest John Perfect for a chairs conference case with a diagnosis of nocardia.  Great work. Today's Question - what is broadbent's sign? Pubmed from the program goes to SAR and Global Health resident Paul Park for his article accepted to Tropical Medicine & International Health:  "Increasing access to the MDR-TB surveillance program through a collaborative model in Western Kenya" Thanks Stead attendings Mitch Heflin and Alastair Smith.

QI Corner (from Dr. Jon Bae) - Update 11/5/2011

Please continue to fill out the Duke University Health System Safety Attitudes Questionnaire (the SAQ).  Our target completion rate is 100% (including med peds and med psych).  We are continuing to improve, but still have a long way to go.  The importance of this survey to understand our culture and to make our work environments and patient care better cannot be understated: you can make a difference! The survey has been sent to you via email and should take between 10-15 minutes.  It will have the subject line "DUHS Safety Culture Survey" from Pascal Metrics (you can search for this). Flu Vaccination

Dates and locations when vaccinations are available are highlighted in the attached flyer.   We'll continue to distribute updated reports through the Stead Society team leaders, who are leading the way in some "friendly" competition to see which group is 1st to hit the 100% mark. What Did I Read This Week  (by Anne Phelps, MD)

[box]“ Depression during Pregnancy: Donna E. Stewart M.D., The New England Journal of Medicine Oct 27, 2011. [/box] Depression is a very common complication of pregnancy affecting more than 12 % of pregnant women.  Untreated depression in pregnancy has been linked to increased risk of suicide, preterm birth, poor fetal growth and impaired post natal development. Mild to moderate depression may be treated with non pharmacological treatments such as psychotherapy and exercise.     More severe depression may require pharmacological treatment.  Although SSRIs and SSNRIs are considered to be relatively safe medications to use during pregnancy, they are not without risk.  Despite their relative safety, SSRIs have been associated with miscarriage, preterm birth, as well as birth defects (including anencephaly, pulmonary hypertension, and cardiac malformations).  Paroxetine and tricyclic antidepressants both have high rates of cardiac malformations, and should therefore be avoided.  There is not enough safety data on desvenlafaxine, buproprion, and duloxetine to recommend their use in pregnancy.  It is essential to thoroughly discuss the risks and benefits with your patient, and document your conversation, prior to starting any pharmacological treatment.

Request for Applications- Resident Research Grants (by Murat Arcasoy, MD)

The annual Faculty Resident Research Grant applications are due on March 2, 2012.  Please see attached the Request for Applications, instructions, a sample NIH style biosketch, and application forms. Please do not exceed the 3 page limit for the Research Plan, and please note that you do not need preliminary data to apply. Our "Resident Research" website will be launched in the coming weeks. These forms will also be posted on the website along with a list of previously funded projects and sample grant applications. To discuss any issues regarding your research interests, for guidance to identify a research project and mentor, for questions related to the application process or once you decide to submit an application, please e-mail me at arcas001@mc.duke.edu.   Please email me as well to declare your intention to apply, your research mentor's name, the title of your Grant Application or any questions. Turkey Bowl Practice

Duke Medicine Fleece (from Matt Chung and the Residency Council)

The opportunity to order a fleece to keep you warm at the "Turkey Bowl" closed on Wednesday.  However, please remember that payment is due no later than Wednesday of this week.  The residency office (Lauren Dincher) has offered to serve as a collection point during regular office hours  (cash or checks only - payable to Matt Chung).   The cost is $25 for the regular fleece, or $30 if you ordered one with your name. On behalf of the residents, faculty and staff - THANK you for spearheading this project!

Reminder… noon conference and grand rounds are being recorded each day. If you missed conference and would like to "catch up", simply open MedHub, go to your personal calendar, click on the conference you would like to view, and then click on the link provided. For past dates, click on “view my calendar” at the bottom of your calendar section, go to the desired date and click on the conference topic.  If you need assistance, please contact the med res office.  Conferences are typically uploaded within 24 hrs. of the actual activity.

SAR Talk Evals

Please remember to pick up a paper evaluation at lunch, complete it and turn in at the end of the SAR Talk.  If you forget, blank evaluations are always available in the med res office.  Feedback is very much appreciated by our SAR speakers.  Thank you in advance.

BLS Blitz – Do Not Miss this Opportunity !

If your certification expires prior to April 1, this is your only opportunity to get the course for free at Duke.  Sessions will be held November 14 through 17 as follows: 8:00 a.m. – 10:00 a.m. 3:00 p.m. – 5:00 p.m. 10:00 a.m. – 12:00 p.m. 5:00 p.m. – 7:00 p.m. 1:00 p.m. – 3:00 p.m. Additional sessions on November 14 and 16 will be held  7:00 p.m. – 9:00 p.m. Location: Hock Plaza, Suite G07 You need to register 24-48 hrs. in advance to reserve a seat. Attached please find the directions to register. Once registration has been completed in Swank, a confirmation will be e-mailed to the address provided. Participants must bring confirmation e-mail and present it at the registration desk. Walk-ins will be accommodated only as space allows.  You must bring a copy of the NEW AHA BLS Provider Manual and your Duke ID Badge.  Books are available to sign out in the med res office.
