The Advance Care Planning (ACP) initiative at the Duke Outpatient Clinic is off to a great start. Patients are already coming to ACP dedicated appointments at the DOC to meet with our new patient navigators to begin “Care Planning,” the patient-centered process of speaking with loved ones and the medical team about the care a patient would like to receive if unable to speak for him/herself.
We have two new patient navigators dedicated to ACP: Indhira Udofia, who started her role as a ACP Patient Navigator in July, and Chenita Jackson, who will be starting later this month in a similar role. The navigators are available throughout the week and are a wonderful resource if you should have any questions around the ACP process.
Do you have a patient that you feel would benefit from meeting with an ACP navigator? Our patients are scheduled for appointments to see our ACP patient navigators by phone or we can ask in-person if they are in clinic. You can send a Maestro care staff message to Azalea Kim to make the referral. You are also very welcome to flag down Indhira or Chenita to see if there’s room in their schedules that day to see patients.
