Claude Piantadosi, MD, professor of medicine (Pulmonary, Allergy and Critical Care Medicine) and pathology and director of the Duke Center for Hyperbaric Medicine and Environmental Physiology, discussed the saga of a fisherman who says he survived at least 13 months at sea in an open boat. Matthew Sparks, MD, medical instructor (Nephrology), commented on new Canadian guidelines on dialysis, saying the initiation of dialysis, "needs to be individualized, not simply generalized by an eGFR value." Sparks said clinicians in the U.S. make the decision to start dialysis when patients develop complications that are refractory to medical management, including hypertension, fluid overload, electrolyte or acid-base derangements, or symptoms of uremia.
