Pearls from 11/16/21 LEADS

The November 16, 2021, session of Duke Medicine LEADS featured Noppon Setji,MD, Jennifer Phillips, Vonda Capps, Kara Lyven, presenting "Understanding Why - The Root Cause Analysis Process: An Overview and Simulation." 

  1. A Root Cause Analysis (RCA) is a type of safety event analysis that takes a multidisciplinary, step-by-step approach through the timeline of actions that occurred during an adverse event in attempt to identify systemic causal factors that contributed to patient harm.
  2. The goal of an RCA is to understand the details of an event, identify contributing factors, and develop an action plan for process improvement. An RCA does not aim to identify or place blame or fault.
