From the Director
The days are getting longer, and it's nice to have warm weather and sunshine into the later evening! A lot happened this week, and there are some important program work about to happen, so bear with me if this is a little bit long.
First, congratulations to JARs

Amy Newhouse stumped Eileen and I on a case of a heart transplant patient with a fever (AML! Who knew!) as well.
We also had a great debriefing of the Safety Attitudes Questionnaire (program director fail for having to go to another meeting during the noon conference!)…
Jon Bae and others are brainstorming ways to improve our resilience, so please talk to him or I if you are interested in getting involved.
Huge Kudos to SAR
Adia Ross and the DOC team for a flawless strategic planning session at the DOC. Other participants included JARs
Newton Wiggins, Phil Lehman and Dale Okorodudu, and Interns Kevin Shah and Jeremy Gillespie. I learned a lot during the session and feel confident that we will be prepared for Maestro.
The last quarter of the academic year is also known as SURVEY TIME! Please get ready, because a number of VERY IMPORTANT surveys are about to come your way…
- ACGME Resident Survey — for medicine residents, this comes out on 4/16. This survey is very important to us and to the RRC. We need at least 80% participation to be in the good graces of the RRC. We will be talking more about the survey on 4/11 at noon conference.
- Annual Program Survey — This comes from my office and is used to help us learn how we can do better, and what we are doing well. It's another chance to give us feedback on the leadership team and the program.
- The Learner's Perception Survey — this is a survey of your continuity clinic. We've collected this data for the past 5 years, and it has been the force behind improvements in your clinic experience.
We'll try to stagger these so you don't get "survey fatigue" but PLEASE help us make OUR program better by participating. As always, they are anonymous!

Handoffs in the Era of Duty Hours Reform
Why I read this? The title caught my eye when I received the email with the table of contents of the April 2012
Academic Medicine. I saw the authors were Duke trainees and faculty, including current IM resident Tian Zhang, Hospital Medicine faculty Brian Griffith and Alyssa Stephany, and Med-Peds alum Priya Gopwani. Congratulations to them!
What they found? The ACGME added the requirement of training in transitions of care to the Common Program Requirements in July 2011. In response, the authors as members of the Duke GME Patient Safety and Quality Council (PSQC) conducted a focused review of the current literature to determine best practices and develop a plan for Handoff education and evaluation for all Duke GME trainees. Their literature review identified 102 articles (all included in their references for anyone who is interested) including 4 comprehensive reviews and the remainder focused on handoff structure, evaluation, and education.
The authors also described Duke’s institution-wide response to the change in requirements, including the creation of the handoff core content guidelines, a plan for educating the residents, and the development of a Handoffs CEX tool (created by the Medicine QI group!).
What I learned? The new ACGME requirement around transitions of care provides programs another method to train residents to improve both the quality and safety of patient care. Duke GME addressed this requirement successfully through a resident-driven process that was very effective in leading change within the health system.

Nephrology Interest Reception
The Division of Nephrology will host a reception for all Medicine House Staff with an interest in possibly pursuing fellowship training in Nephrology on Friday April 27, 2012 from 5:30 – 8 PM at the home of Dr. Rudy Lehrich. This will be a low key setting to obtain information from current fellows, our current SAR’s who will be joining the program in July, and a handful of faculty. Questions with Audio Enhancements Added to Summer 2012 Internal Medicine Certification Examination
Take note that ABIM has added something new to the certification exams - a small number of audio-based questions that will require you to recognize and interpret recorded heart sounds. You will listen to the sounds using headphones supplied at the test center.
How to prepare for your exam:
- Schedule your exam - If you have not done so already, schedule your exam by contacting Pearson VUE: www.pearsonvue.com/abim

- The updated tutorial will also feature how to use highlight and strikethrough capability when reviewing all exam questions.

In addition, after reviewing the abstracts three finalists will be chosen to participate in a Young Investigators Competition and will present their work orally on the day of the symposium (they will be notified around April 9th in order to have time to prepare). A winner will be announced at the end of the day.
