Sharing DOC Clinic Success – Bowlby Invited as Speaker

Lynn Bowlby, MD, Medical Director of the Duke Outpatient Clinic (DOC), has been invited to speak about the results of this redesign at the Partnership for Patients Education Series on February 24th in Irving, Texas. The series is entitled Reducing Health Disparities: Hospital Success Stories.

You may have heard, the DOC recently underwent a redesign for improved population health management and resident education. As a result, this “best practice” was picked up by the Hospital Engagement Network (HEN), a consortium of hospitals charged by CMS to share the news with other hospitals. The HEN goal particularly is to reduce avoidable readmissions.

This series will be all about teaching healthcare professionals how fully understand the unique needs of the patient populations they serve by collecting accurate and consistent REAL (race, ethnicity and language) data. Click here for more information on the series. 

Bowlby will discuss how the clinic was able to decrease the patient population for whom data about race and language was “unavailable” from 20 percent to 8percent from November 2006 to March 2008 and identified disparities in cardiac health in the patient population. This focused effort resulted in the same percentage of African-American and White patients receiving appropriate heart failure and heart attack care. She will also talk about how they have been able to take the next step and redesign the Duke Outpatient Clinic to improve access to care while lowering costs and improving the quality of care for their patients.
