We are excited to announce the Duke Outpatient Clinic as recipients of two recent awards:
3D IT TAKES A TEAM AWARD
One award was a quarterly 3D It Takes a Team Award presented by Duke University Hospital's Chief Operating Officer, Carolyn Carpenter. The DOC was selected for this award because of their "TAGTEAM" service, a "Team Approach Geared Toward Effective Anticoagulation Management". In 2011, the DOC pharmacy services took over warfarin-anticoagulation management for patients at the DOC. They quickly discovered the challenges in such a highly complex patient population, so they developed a process to review management for each patient during monthly leadership team meetings.
Carpenter stated in her award announcement:
"Your story was an excellent example of how we Discover, Develop and Deliver [3D] a healthier tomorrow for our patients, their loved ones and each other."
STRENGTH HOPE AND CARING - TEAM AWARD
A tribute to their compassionate, thoughtful care coordination that focuses on putting the person who needs their care at the center of everything they do, including how they came together to coordinate comprehensive services for a patient who was homeless.
