Pearls from 9/21/21 Medicine Leads

The September 21, 2021 session of Duke Medicine LEADS was presented by Rebecca Sadun, MD, PhD, Robert Benjamin, MD, Carly Kelley, MD, MPH and David Ming, MD on  "Improving Care Transitions at Duke: How Can We Bridge the Gap?."

Here are pearls and takeaways from the session:

1. There are several helpful resources that can aid in the pediatric-adult care transition. Some of them are: 
   • The “Six Core Elements” which can be found at gotransition.org

2. In patients with complex or niche medial needs, there is often a paucity of well-defined quality measures. This may contribute to care fragmentation. 
3. It is best to think about coordination of care, including during a period of transition, as an ongoing and evolving process rather than one that is finite and discrete.  
4. When thinking about care coordination, always consider family and cultural integration. Care coordination can become uncoupled if not performed with the family in mind.
