- There are several helpful resources that can aid in the pediatric-adult care transition. Some of them are:

- The ACP Pediatric-to-Adult Care Transitions, Condition-Specific Toolkit which can be found at https://www.acponline.org/clinical-information/high-value-care/resource…;
- In patients with complex or niche medial needs, there is often a paucity of well-defined quality measures. This may contribute to care fragmentation.
- It is best to think about coordination of care, including during a period of transition, as an ongoing and evolving process rather than one that is finite and discrete.
- When thinking about care coordination, always consider family and cultural integration. Care coordination can become uncoupled if not performed with the family in mind.
