The Advance Care Planning (ACP) initiative at the DOC is off to a great start. Patients have already started coming to ACP dedicated appointments at the DOC to meet with our new patient navigators to get started on “Care Planning,” the patient-centered process of speaking with loved ones and the medical team about the care a patient would like to receive if unable to speak for him/herself.
